Release of Information
In case there is a need to send or make patient medical records (including imaging) available to a non-Loma Linda University Health physician/provider, please send the following information either via FAX or email. Note that all items are required except where noted (essentially only the email addresses are optional):

- Email address (where applicable)
- Information to be released:
- Patient Name:
- Patient MRN:
- Patient DOB:
- Study or Studies to be released (date and name of each study):
- Method of release (hyperlink, FAX, paper, etc)
- Note that for imaging studies, Health Information Management is able to send an encrypted link to an email recipient who can then access the images from our PACS system. Images can be viewed via the link or can be downloaded for purposes of import to another PACS or DICOM viewer.

Regular hours of support are:
- Monday-Thursday 8:00 AM to 5:00 PM
- Fridays from 8:00 AM to 4:00 PM
- Closed on recognized holidays and weekends
For Urgent requests outside of regular hours, please contact the Hospital Operator and ask for the on-call PACS personnel.
