Our Purpose
The Health Information Management Department recognizes the vital role of healthcare information. We are committed to providing the highest quality of service to our customers by ensuring the accurate, dependable, and secure collection, maintenance, and appropriate dissemination of patient information regardless of its format.
- We are committed to the:
Organization and maintenance of patient information - Advance electronic health records by digitizing inbound paper records
- Quality of Diagnosis and procedure coding for inpatient and outpatient services
- Release of patient information as authorized by the patient or as mandated by state and federal guidelines
- Reporting of statistical information
- Maintain internal systems and support their utilization through ongoing training
- Organization of reviews of patient information to help identify areas of improvement in patient care

Hours of Operation
LLUAHSC Support Services Building (LLUAHSC-101)
You can access your health information by accessing our patient portal, MyChart. If you are unable to find the information you’re looking for on MyChart, you can request your medical records. We are electronic so visitors are being seen by appointment only.
Business hours for Release of Information
Monday - Thursday: 8 a.m. - 12 p.m. & 1 p.m. - 4:30 p.m.
Friday: 8 a.m. - 11 a.m. & 12 p.m. - 3 p.m.
