2024 Hospital Week Art Contest

Department Heads will pick-up the art file from their hospital site.

Sites will be listed on Leadership Connection.

• Department Leaders will choose and pick up a doodle art poster for their area (pick up sites for each hospital will be listed in Leadership Connection).
• Place this poster in an area that all employees can access. (poster putty will be provided). 
• Submit a photo of your completed art no later than May 17.
• A committee will consider the votes and make final selections. 
• Winners will be provided lunch for their area (three winners will be selected).

Hospital Week Photo Contest Terms & Conditions

• Please adhere to LLUMC policies:
  • E-12 'Information Classification and Protection, Section D (4)
  • LLUMC A-22 Photography, Videography, and Audio Recording
• The picture must NOT include patients and/or identifiable patient’s protected health information (PHI).
• The picture must NOT have PHI in the background (such as computer monitors or boards in the background displaying patient information).
• The picture must NOT have patients family, friends, or visitors identifiable in the picture.
• There should not be other employees identifiable in the picture without their prior consent.
• By submitting the picture(s), you and everyone depicted in the picture authorize the use and posting of your image (on the external or internal channels, etc.) and personal information (including name, department, and role as described below).

Authorization to Use and Publish Photographs and Personal Information

By posing for the photograph and submitting it to LLUH during Hospital Week, every individual portrayed in the picture agrees to the following:

1. Subjects of the photograph grant to LLUH or its designees the right to use their personal information such as name, role in the organization, and department, along with their image portrayed in the picture or other media (“Product”), now known or later developed. This grant includes, without limitation, the right to edit, mix, or duplicate the Product in whole or in part as LLUH may elect.
2. Subjects of the photograph hereby grant LLUH and its designees the right to use, reuse, broadcast, rebroadcast, exhibit, market, sell, and otherwise distribute the Product, either in whole or in part, and either alone or with other products, for commercial or noncommercial website, podcast stream, television, theater, closed-circuit exhibition, home video distribution, or any other purpose that LLUH or its designees in their sole discretion may determine.
3. Subjects of the photograph waive any claim for loss or damages that they may have against LLUH and its designees, and they agree that no compensation in connection with these pictures or any other use of their name, face, likeness, voice, and appearance is due, now or in the future.
4. Subjects of the photograph confirm that they have the right to enter into this Agreement, that they are not restricted by any commitments to third parties, and that LLUH has no financial commitment or obligation to them as a result of this Authorization. The rights granted to LLUH and its designees herein are perpetual and worldwide.
5. Subjects of the photograph have read the foregoing, understand its terms and stipulations, and agree to all of them.
