Electronic Event Reporting (EER) - Formerly Gold Event Forms
Electronic Event Reporting (EER) is an easy and effective way to immediately report events related to patient safety, patient care and other unusual occurrences within our organization.
All events must be reported without delay as some events may need to be reported to regulatory agencies within defined time frames. You will be presented with different questions based upon the type of event that you need to report. Please report facts (who, what, when, where, why, and how) to help resolve events involving patients. Please be as accurate and concise as possible.
REMINDER: When looking up patient by number (FIN or MRN), do not enter the leading zeroes. Example, FIN 00123456 should be entered into ID Number field as “123456”.
MEDICATIONS
Medication related event
(e.g.) Allergy, gave wrong medication, expired, missed dose, 5 rights errors, delay in receiving medication, medication labeling error
LABORATORY | BLOOD PRODUCTS
Lab test / specimen related product
(e.g.) Blood test delay, lost, drawn incorrectly, mislabeled, specimen related incident, problem with blood infusion, blood bank concerns
LDA LINES | DRAINS | AIRWAYS
Tubing / catheter event
(e.g.) IV infiltrations, delay in line placement, LDA pulled out, extubations
TREATMENT | PROCEDURE
Treatment / procedure event
(e.g.) Policy and procedure not followed, break in sterile technique, incorrect count, orders placed but not done, surgery/procedures cancelled or delayed
CHANGE IN PATIENT CONDITION
Unplanned admission / return to surgery
(e.g.) Code blue concerns, unplanned admission, return to OR within 24 hours, unplanned death, moved to critical care unit, significant change in vitals, prolonged recovery time
DOCUMENTATION
(e.g.) LLEAP concerns, incomplete orders, incorrect entry on medical record, issues with consents
BEHAVIOR | COMMUNICATION
Behavior communication event
(e.g.) Demeanor or actions that are inappropriate, unprofessional communication, patient agitated, family upset, service not responding to page, left unit to smoke
FALL
Patient fall
(e.g.) Observed fall, unobserved and found on floor
PRESSURE ULCER | ABNORMAL SKIN
(e.g.) New or worsening pressure ulcer, skin abrasions or tears, lacerations, rashes, burns
DIET | NUTRITION
(e.g.) NPO patient ate, wrong diet or formula, food delivered late
LEFT AGAINST MEDICAL ADVICE | ELOPED
(e.g.) Patient left without discharge, no instructions given, walked off unit
EQUIPMENT | MEDICAL DEVICE | SUPPLIES
Equipment / medical device / supplies related event
(e.g.) Defective, broken, unavailable
PROPERTY
Property missing / damages
(e.g.) Missing or lost money, valuables, cell phones, dentures, glasses etc.
