Healthcare employees

Effective August 5, 2023, the Centers for Medicare and Medicaid (CMS) have ended the COVID-19 vaccination requirement for healthcare workers.  Healthcare workers will no longer be required to obtain COVID-19 vaccination. More information can be found at Revised Guidance for Staff Vaccination Requirements.

Non-healthcare employees

Please refer to policy T-80, Healthcare Staff COVID-19 Vaccination for details regarding current policy.

MyChart

If you are a healthcare employee who is vaccinated, please upload proof of vaccination. 

All healthcare employees should contact Number Control, (at 909-558-4000 ext. 58235 or email them at NumberControl@llu.edu) and request that they send you the questionnaire needed to upload your vaccine card.

Please upload your vaccine card by following these steps:

• Create a MyChart account or log into your existing account
• Once logged in to MyChart
• Select the messages icon
• Select the message for vaccine verification
• Follow the prompts
• Tip Sheets
  • MyChart Sign Up
  • Uploading vaccine documentation ( Step-by-step video)

Even if you do not have insurance through LLUH please follow the instructions to setup a MyChart account. The process will be the same for you regardless of insurance.

Please note, submitting photographs of your vaccine record to providers via a MyChart message you create does not qualify. Submissions must be via the auto-generated message from MyChart as described in the tip sheet Uploading vaccine documentation.

If you are a contractor or an employee who is unable to set up your MyChart account or have not received a MyChart notification/message, please call Number Control at 909-558-4000 ext. 58235 or email them at NumberControl@llu.edu.
