Scrubs FAQ
- Scrubs not returned to the system (including lost, stolen, misplaced, missing or abandoned scrubs).
- Credit discrepancy not reported within a 2-week timeframe of the occurrence.
- Scrub returns that can’t be verified by the scrub system (verified returns are successful transaction records or picture taken by the system).
All credit discrepancy must be reported within a 2-week timeframe of the occurrence. Any credit discrepancy over 2-weeks is considered as a scrub loss.
For Maternity Service employees only, contact your department admin secretary/assistant.
For all others department:
If the discrepancy occurred within the last 2-weeks:
- During Business Hours (listed below), call EVS at ext 44326 and ask for the Linen Supervisor or On-Duty EVS Supervisor.
5:30 a.m. – 2 p.m. Mondays thru Fridays - After Business Hours, Weekends and LLUH Recognized Holidays:
- Send an email to cmarifosque@llu.edu (Debbie Marifosque) and cc mfsilva@llu.edu (Maria F. Silva) and amayo@llu.edu (Alex Mayo).
- Please include the following information:
- When did this happen (date and estimated time)?
- The location/department/campus where issue occurred (ie: 3rd Floor / OR / TMC).
- How many items did you return and/or a short description of what happened?
- If you received an error message, please also include that in your email.
If you received an error message: Email the complete error message, along with any numbers that show along with the error message and email to cmarifosque@llu.edu (Debbie Marifosque) and cc mfsilva@llu.edu (Maria F. Silva) and amayo@llu.edu (Alex Mayo)..
If you have received a badge replacement. Access does not transfer to replacement badges. You will need to notify us via email and your new badge # will need to be programmed into the scrub system(s).
- Send an email to cmarifosque@llu.edu (Debbie Marifosque) and cc mfsilva@llu.edu (Maria F. Silva) and amayo@llu.edu (Alex Mayo).
- Provide the badge # on your badge.
- Vertical Badges (old): # is located on the back, underneath the black stripe, bottom right-hand corner.
- Horizontal Badges (new): # is located on the back, first 6-digits in the number string at the bottom of the badge)

NOTE: If you are unable to read the Badge # due to wear and tear, please call HR (ext 14001) and request for your badge #.
EVS/Linen Department provides access to the scrub machines only. We do not provide access to doors, entryways, locker rooms, lockers or medicine cabinets.
- For door access (including locker room access): Your department or school will need to send a request to Door & Access Services.

- Need to access scrubs to get into the OR as soon as possible:
- For OR employees ONLY:
- Go to the OR Charge RN at the OR Front Desk and request for a set of scrubs (they’ve been provided a scrub badge with extra credits or have several “super users” who have been provided extra credits and they should be tracking the credits to have them readily available for OR employees per Ilsa Nation, Thelma Chaffee and Linda Puma).
- During business hours: Call EVS at ext 44326 and ask for the Linen Supervisor.
- After business hours, weekends and LLUH recognized holidays: Call EVS at ext 44326 and ask for an EVS Supervisor (they are available on all shifts, 24/7). They can loan a set of scrubs. They will request for your name, badge # and EID #.
- Go to Central Service and request for a set of disposable scrubs. You will need to provide your department’s 5-digt cost center (your department admin secretary/assistant can provide this information). See Central Service locations below:

- For Residents:
- During business hours: Call EVS at ext 44326 and ask for ask for the Linen Supervisor..
- After business hours, weekends and LLUH recognized holidays: Call EVS at ext 44326 and ask for an EVS Supervisor (they are available on all shifts, 24/7). They can loan a set of scrubs. They will request for your name and badge #.

- For Students/non-OR employees:
- During business hours : Call EVS at ext 44326 and ask for the Linen Supervisor.
- After business hours, weekends: Go to Central Service and request for a set of disposable scrubs. You will need to provide your department 5-digit cost center (your program coordinator can provide this information). See Central Service locations below:

- For OR employees ONLY:
- If you recently had a status change (change in occupation and/or new department – for example: you are an employee who transferred to a new department or you were a student, but you’re now a resident). You will need to have your dept admin secretary/assistant or program coordinator contact us to re-activate your account and update it with your new information (badge #, cost center, and occupation).
- If you are a student and had access before, but your badge no longer works. As a student, you are given temporary access during OR rotations only (does not include voluntary rotations or shadowing a physician). Your access ends at the end of each rotation and your access can only be re-activated or extended upon request by your program coordinator.
- Contact your program coordinator to request a re-activate access or extension.

For students on voluntary rotations or shadowing a physician, scrub access is not provided. Go to Central Service and request for a set of disposable scrubs. You will need to provide your school’s 5-digt cost center (getthis #

Completing the self-registration process does not automatically give you access.
- For verification purpose, your access must be submitted by a designated scrub contact (either your program coordinator or your dept admin secretary/assistant).
- Scrub access must be pre-approved by an OR designated approver(s).
- You will also need to have reviewed, signed and returned a Scrub Usage Guideline form. Forms must then be sent to your dept admin secretary/assistant or program coordinator. They will need to submit it to EVS to process your registration.
NOTE: Self-registrations in TMC will not be processed without a signed Scrub Guideline and an OR department’s pre-approval.
Contact your dept admin secretary/assistant or program coordinator. You may have an account in the system but may be disabled. Your dept admin sec/assistant or program coordinator will need to contact us.
- Call EVS at ext 44326 and request for the AM or PM Linen Supervisor or an On-Duty EVS Supervisor (we have supervisors on every shift 24/7). They can arrange to loan out a set of scrubs. They will ask for your name badge # and EID #. Any scrubs loaned out will need to be returned to EVS within a week of the loan or charges for the scrubs will be charged to your department or school
Student scrub access are temporary and are only provided during formal OR rotations. Scrub access disables at the end of each rotation and are only re-activated if requested by their program coordinator. Once scrub access is disabled, that means you can no longer dispense or return scrubs.
Contact your program coordinator as they will need to send a request to our department.
