Clinical Forms Committee
Our Purpose
To provide guidance and consistency in the development, review and approval of all clinical forms that will become part of the patient’s medical record, including paper and electronic.
Our Responsibilities
To establish a focused review of clinical forms. Review includes:
- Encourage standardization and minimize multiple similar forms.
- Confirm documents comply with regulatory/statutory requirements and LLUH policy and standards.
- Identify appropriate logo.
- Consider if form needs Legal review. All consent forms require Legal review.
- Determine if form needs Health Information Systems Committee (HISC) review, which meets monthly.
Our Committee
The Clinical Forms Committee meets every other month, on the first Wednesday of the month, beginning in February. For any questions or concerns, email HIMClinicalForms@llu.edu
