This is a library of clinical forms provided for quick access to these key resources.

NOTE: California State Health and Safety Code 123222.1 requires that printed materials provided to the patient, including consents, must be printed in at least a 12-point font that is clear and legible.

Since individual printer types such as HP, Brother, Epson and others do not have the capability to print in an exact minimum 12-point font size, Loma Linda encourages each department to order consents and specialty paper forms from Printing Services. 

Specialty paper forms include bi-folds, tri-folds or duplicate copies (NCR type) that are distributed to the chart, patient, pharmacy and nurse. These specialty paper forms are included in the Clinical Forms Library for your reference only.

Tips:

• Looking for a specific form? You can either open individual folder(s) for the type of form you are looking for, or you can click “expand all” under each section (i.e. LLUH Clinical Forms, CH Clinical Forms) and all of the folders in that section will open. You can even “expand all” of the folders in all of the sections to expose all forms for a search. Then simply use the Find feature by using Ctrl+F to search for a word in the form name, or the form number.
• To determine the revision date of a form, please click the link to open the form in Adobe Reader, scroll down to the bottom of the page and look at the bottom of the page in the middle of the form. The revision date will be posted with the form number as follows:
  “23-0043B (9-11)” where 9 is the month and 11 is the year.
