  1. Where can I find applications/amendments/forms?
    A: Applications, amendments and other forms can be found on the Forms page under the  Radiation Use Authorizations button or by clicking on the Forms link on the Management of Radiation Generating Equipment page. Forms can also be obtained by contacting the Office of Radiation Safety by email or at extension 14003.
  2. When is the next training class?
    A: Radioactive Material Safety, Cobalt Irradiator and Radiation Therapy Safety training are provided quarterly.  For dates, times and other training courses, contact ORS at extension 14003.
  3. I need a radioactive waste pick up…
    A: Contact the Office of Radiation Safety by email or at x14003 and choose option "3"
  4. Where can I find radioactive waste containers?
    A: Containers needed for patient use can be found in the biohazard rooms located on each floor/unit. Containers needed for research areas can be obtained by contacting Office of Radiation Safety by email or at x14003 and choosing option "3"
  5. What forms do I need to complete to use radioactive materials?
    A: Applications and amendments can be found on the Forms page under the Radiation Use Authorizations button or by clicking on the Forms link on the Management of Radiation Generating Equipment page.

    For new researchers or new research protocols involving non-human use of radioactive materials, complete a "General Application." For currently authorized users or current research protocols with minor changes, such as change in isotope, chemical form, location, or personnel, complete the "Application for Amendment". For human use protocols, complete the "Application for Human Use of Radioactive Materials and Ionizing Radiation". For protocols involving animals and radiation, the Office will use the IACUC application.  Have all individuals complete the appropriate "Statement of Training & Experience" form.

  6. I need a dosimeter. How can I get one?
    A:
    Contact Office of Radiation Safety by email or at x14003 and choose option "0"
  7. Can I submit my application electronically?
    A: Yes. You may email your completed forms and attachments to Radiationsafety@llu.edu
  8. I’m pregnant. What about training and a film badge?
    A: For information, click on the "Training Information" button and then the "Pregnancy and Radiation" link on the page. If you have questions, contact the Office of Radiation Safety by email or at x14003 and choose option "0".
