Requesting Data for Patient Care and Treatment

Patient data needed to facilitate patient care and treatment can be accessed and obtained in the same manner that it was accessed and obtained prior to implementation of the Privacy Rule. The Privacy Rule does not impose or restrict a healthcare professional's access to information necessary to provide timely, quality care to the patient. As such, requests for patient data for the purpose of care and treatment must be provided in a timely manner to prevent adverse clinical outcomes.

Requesting Data for Billing and Payment

Patient data should be shared within the LLUH OHCA with departments and personnel authorized to receive billing information or departments/clinics that must receive patient data in order to bill for services they rendered to a patient. Minimum necessary requirements apply to the sharing of patient data for this purpose. Only patient data related to the bill(s) or claim(s) in question must be requested or provided.

Requesting Data for Other Activities

When the data you need is held outside your department
To obtain patient data for healthcare operations (which includes training and education) or for research activities, request the data by filling out a Data Request Form and submitting it as an attachment to a Service Request at Service Portal. The Data Request Form may also be submitted directly within the Service Portal by selecting “Request Products and Services” from the main page and then selecting “Data Analytics” and “Data Intake Request”.
