Reporting Violations or Possible Violations

To Report HIPAA related concerns, you may contact:

• For privacy breaches or vulnerabilities that need to be addressed through operational processes, policy or procedures: The Office of Corporate Compliance, x14200 or via the Report of Concern online form 
• For threats to computers, application and systems or technical questions: Information Services - Desk Service Desk (MC): x48889; LLU: x48611 
• For threats to facility security and housed equipment e.g., theft or unauthorized access: Security Department, x44319

Reporting Anonymously

If you wish to report a violation anonymously, you can contact the Compliance Reporting Line (Hotline) at 800-249-9953 or submit your report securely via the Compliance Reporting Online Form. The Compliance Reporting Line is staffed by individuals who are not affiliated with LLUH. Therefore, calls made to the toll-free Reporting Line number and reports submitted via the online form are not traced back to the individual making the report.

When reporting anonymously, please provide detailed information so that a thorough investigation can be conducted.
