GoCheck Kids – Improving vision screening from 1-4 Dr. Alexandra Clark This QI projects long term aim is to create a standardized nursing and physician workflow coupled with a technical solution to screen >90% of children between the ages of 1-4 with age appropriate vision screening by one of 3 methods; Snellen, LEA chart, or GoCheck Kids photo screening tool when they present for their well child check in the months of September thru December 2019. During the pilot phase and subsequent PDSA cycles, this project aims to screen >90% of the children between ages 1-4 years who are being seen by the physicians assigned to the pilot lead MA in each PDSA cycle week.  A secondary aim of the project is to refer >95% of abnormal screens to pediatric ophthalmology/optometry. 6/21/2019

SAC SBC Pediatrics Influenza Vaccine Patient Education & Counseling Dr. Matthew Fong

The project aimed to increase the percentage of patients who initially refused to receive the influenza vaccine but then agreed to receive it by 20% over baseline by the end of the influenza season.

Main influenza vaccine project

This project was part of a larger, clinic-wide project to increase influenza vaccination administrations aimed to increase the number of influenza vaccine administrations during the 2018-19 flu season to 5000 as compared to approximately 3600 given the previous season. Additionally, the project aimed to increase the opportunity % of influenza vaccine administration to 70% during the peak influenza vaccine administration months of September 2018 to February 2019.

Improving Recognition and Documentation of Childhood Hypertension QI Project at RUHS General Pediatric Clinic Dr. Oluwafisayomi Nwachuckwu The specific aims of this study are to increase by 10% the recognition and documentation of elevated BP in adolescents (13 years and older) in our clinic (RUHS Gen Peds Clinic), and to increase by 10% the number of follow-up clinic visits documented in electronic health record (EHR) for BP recheck for these adolescents by August of 2019. 2/25/20
Using Pulse Oximetry to Decrease Length of Stay in Hospitalized Children with Acute Respiratory Illness, A Quality Improvement Project Dr. Gabrielle Balan The primary aim of our project was to decrease LOS for pediatric patients admitted to the pediatric ward at RUHS with the diagnosis of acute respiratory distress (see below) diagnosis for patients between age 30 days to 18 years of age by 20% by decreasing unnecessary continuous pulse ox usage amongst these patients by April 2019 with evidence of sustained continuous pulse ox requirements by April 2020. 11/26/19
Standardizing the Care of Hospitalized Patients with Asthma from Door to Door Dr. Ekua Cobbina To standardize our treatment and weaning of therapy inpatients admitted in with asthma exacerbation to decrease our lengths of stay by 20% over 2years and meet national benchmarks. 10/23/2019
Ensure CDC Guidelines for the administration of PPSV 23 Vaccine for children residing at TK. A long term care Rehab Hospital/Subacute Facility Dr. Ravindra Rao

This project has 2 primary aims.  First, to identify the children who need to be vaccinated with PPSV23 or Prevnar 13 who reside at TK and complete at least 80% of vaccination gaps in a period of 6 months. Secondly, this project aims to create a new admission workflow to identify children within 30 days of admission who are in need of PPSV23 or Prevnar 13 and complete 100% of the needed vaccines within 10 days after identification.

Secondary aims included nurse education about vaccine safety as well as tracking any vaccine side effects.  The goal was to achieve >80% nurse education before vaccines were administered and tracking of side effects for 48 hrs in 100% of patients that received a vaccine.  Side effects that were monitored for were: 1. Fever more than 100.4 up to 48 hrs after the vaccine; 2. Development of any local reaction, i.e. redness at the site of the injection; 3. Extreme irritability, manifested with excessive crying and increased heart rate; 4. Document an use of Tylenol/Motrin in the 48 hrs following the vaccine

Anemia Screening and Follow Up in SAC SBC Peds QI Project Dr. Neha Mitragotri

The project aims to improve the percentage of abnormal screens that are correctly identified as abnormal, that are appropriately worked up using a standardized management pathway, and that have appropriate documentation of dietary and family history over 1 year at the SACHS pediatric clinic. The project’s intermediate aim is to correctly identify 80% of abnormal screens, and to achieve 75% appropriate management (as defined by our management pathway) by January.

Increasing Attendance Rates Within Our Multidisciplinary Pediatric Obesity Clinic

Our pediatric obesity clinic at RUHS had a high number of no shows, and very few patients completing the entire 8 weeklong courses. Pediatric obesity is an epidemic that should be approached in a multidisciplinary fashion. Our obesity Clinic’s, which is called Jam Time as to not associated obesity with healthy lifestyle changes, goal is to cultivate lifestyle changes for long term success. In this QI project, the goal is to have 50% of all patients return to at least 50% of all sessions offered by January of 2020. 7/20/2020
One Minute Preceptor Faculty Improvement Project at Social Action Community Health System Pediatrics

Quality resident teaching involves consistent constructive and reinforcing feedback from faculty to residents. The One Minute Preceptor is a teaching tool that provides structured feedback. This project seeks to increase the constructive and reinforcing feedback that faculty provide to residents during chiefing in the general pediatrics clinic at SACHS.  Prior to this project, the faculty did not have a standard for precepting residents, so the quality and consistency of feedback was poor. 8/10/2020
Improving Management of Anemia in a Residency Teaching Clinic Dr. Kelly Jeu By May 2021, we will standardize the approach to management of anemia in a resident ambulatory clinic which includes correctly identifying anemia, ordering the appropriate workup, and gathering appropriate history for risk factors of iron deficiency anemia. We will accomplish these three goals in 85% of visits when children are screened for anemia.  4/16/2021

Improving resident evidence-based medicine presentations at SACHS Dr. Matthew Fong By the end of the academic year we will improve the quality of resident EBM presentations (measured using a scoring tool) by increasing monthly average scores to 80% or greater for three consecutive months, and we will increase the number of EBM presentations with sufficient audience participation (as defined by discussion by 3 or more persons) to 75% for the month. 6/18/2021
Improving Synagis prophylaxis for RSV eligible discharged from the NICU Dr. Jacqueline Bierwirth, Dr. Larry Ngo, Dr. Benjamin Harding

Our quality improvement project aims to improve the identification of eligible patients and communicating it with the community providers by including information in the discharge summaries of our NICU patients.

 

Our specific aims are:

  • Primary goal: Increase the inclusion of RSV eligibility eligible infants’ discharge summaries to >90% within 6 months

Secondary goal: Increase the Synagis prophylaxis prior to discharge completion to >80% within 6 months
