Radioactive Material User Responsiblities
As a user of radioactive material at Loma Linda University Health, you must fulfill various record-keeping and performance responsibilities that will enable our institution to comply with the specific conditions of our California Radioactive Materials License and applicable regulations. The LLUH Radiation Safety Manual describes your responsibilities. Some of the requirements are as follows:
- Amendments and Applications: Authorization is given only for the specific isotopes, compounds, activities, and purposes specified on each approval. Any change that requires a different location, different isotopes, different chemical forms, higher possession limits, or a change in the procedure itself, will require either an amendment or a new application. Check with the Office of Radiation Safety (ORS) if you have questions about which form is required.
- Radioisotopes in Stock Report: This report will be sent to you from ORS each month and must be completed and returned promptly. A decayed total activity of the radioisotopes that you have purchased and that were reported for the previous month is generated. You must report your current inventory by either confirming or updating the total.
- Renewal of Radiation Use Permit Report: This form comes from ORS annually before the anniversary date of your original authorization and must be completed, either reaffirming your authorization for the next year or cancelling it. Any changes in personnel, use locations, protocols, etc., must be noted. Please return this form promptly.
- Periodic Lab Inspections: Your laboratory will be periodically audited by a representative of ORS. All required records must be available for review at the time of the audit. A list of the required records is available in the Radiation Safety Manual.
- Radioactive Material Purchase and Receipt: ORS must be notified of all purchases and transfers of radioactive materials prior to an order being placed. Package opening surveys must be performed and recorded as described in Appendix V of the Radiation Safety Manual.
- Contamination Surveys: Contamination surveys must be performed and results recorded as described in Appendix VI of the Radiation Safety Manual. Any spills or contamination events must be documented along with any corrective actions taken.
- Radioactive Waste Disposal: A record of all radioactive waste picked up by ORS must be maintained. See the Radiation Safety Manual for a description of radioactive waste disposal requirements.
