Schools

Health care professionals completing immunization records, school or sport physicals and illness certifications must obtain a valid written authorization from the patient or patient's legal representative prior to sharing or releasing information to schools. School related health information may be released to the patient or the patient's legal representative providing the release is documented in the patient's chart. Any requests for information unrelated to school activities or information that exceeds five pages should be forwarded to the Health Information Management (HIM) department for processing.
