A: Please refer to the flow sheet 'LLEAP Functionality'. - How can I request a copy of my medical record?
A: We can mail, fax or email to medicalrecords@llu.edu an Authorization Form. You may also print one from this website under the forms section. - Is there a charge for me to receive my medical records? What form of payment do you accept?
A: There is a charge of $0.25 per page for personal use, plus any additional postage fees. An invoice will be mailed to you prior to the medical records being mailed. As a courtesy, there is no charge if your medical records are sent to Health Care Provider. We accept cash, check, money order, and credit cards. For your security, credit card payments must be made in person. - May my spouse request and receive copies of my medical record?
A: Only if he/she is the Executor or has Power of Attorney over your Healthcare. Otherwise, no they cannot. - What are the hours of operation?
A: Monday-Thursday 8am-5pm, Fridays 8am-2:00pm - Will you fax copies to my home or office?
A: We do not fax medical records to patient’s homes or office. We will fax to your doctor’s office or hospital if it is required for patient care. - Will you mail the records or will I have to pick them up?
A: We will mail the records, unless other arrangements are made in advance with us. - How long do you keep my records?
A: At this time, indefinitely. - How may I request copies of records from my last doctor visit?
A: An Authorization Form is required. Access them online either the English ROI or Spanish ROI forms. - May I obtain a copy of my x-ray films from you?
A: Yes, you may also request a copy of you radiology images that will be provided to you on portable media (CD/DVD). The reports that interpret the study can also be provided upon request. - When may I expect to receive a copy of my records?
A: The law allows us 15 calendar days to answer a request. - Where are you located?
