• Download DAISY Nomination Form— we also have paper nomination forms available for your convenience. These nomination forms are located in patient care areas. You can drop your nomination at a designated drop box located in patient care areas or you can scan the form and send it to NurseRecognition@llu.edu
• DAISY QR Code and Nomination Website — this printable flyer contains a QR code and a URL that will take you to an electronic nomination form. This online form can be accessed from anywhere from any device. This option is best for patients, or staff that would like to electronically submit a nomination off campus. You may also print this flyer and post on your units, departments, or hand out to your patients during their stay or on discharge!
