  • Intensity
    • Patient’s self report pain score
  • Quality / Description
    • e.g., Sharp, Throbbing, Shooting.  This information is helpful for diagnosing the Type of Pain
  • Frequency
    • Is the pain constant or does it come and go?
  • What makes the pain better?  Worse?
  • Onset
    • When did the pain start? 
  • Pain’s effecting on function and quality of life
    • Does pain keep you from doing something you would like to do?   
  • Is this pain new or different from earlier pain assessments?
