Expense Report & Travel
The Audit Committee meeting, the Committee expressed an expectation that all expense reports submitted will fully comply with LLUH policies, the IRS and other reporting guidelines. As such, we want to ensure you had the information necessary to prepare a complete and accurate expense report.
Within this section you will find:
- A list of applicable policies. Policies are anticipated to be updated during 2012, so please ensure you periodically review VIP for any changes.
- self-review checklist. This checklist represents the critical criteria that will be used by finance staff and internal audit to evaluate whether the expense report is deemed in compliance with policy and audit committee expectations.
- A request for information form. In the event that finance staff needs additional information, your expense report will be returned with this form requesting additional information.
- An affidavit form for instances in which the original receipt is lost.
- A summary of IRS regulations related to expense reports under the IRS accountable plan rules (IRS Publication 463.)
As the preparers of the expense report, you are the organization’s first line of ensuring compliance. We appreciate your diligence in this process as together we strive for 100% compliance.
Your organization’s controller, financial officer, and/or internal auditors are all here to assist you will questions or concerns.
