Working and living in the communities that we do, we have a continual chance of coming in contact with people who have TB (Tuberculosis). When we get patients in the hospital who have suspected or confirmed active TB, in order to discharge them to the community, the TB Discharge Form must be filled out and permission to discharge received from the County Health Department.
