- If treatment is being sought, complete the DWC-1 form.
- As directed by the nurse advisor, you will seek medical care at one of the following locations:

- Do not go to your private physician or to any other medical clinic when you have been injured or become ill due to your work.
- If you require transportation to the Occupational Medicine Center, you may contact Dispatch at Ext. 44350.
- If you are working in a clinical environment, please do not request consultations from physicians in your department.
The Workers Comp Center will submit a Request for Authorization form for all treatment except surgery to the carrier for the recommended treatment. Please allow 5-7 working days to receive a status.
Please contact your assigned Workers’ Comp claim’s adjuster at (909) 651-4104.
Please take these forms to your Primary Treating Physician’s office for completion. (Please note each office may have a fee and waiting period for such forms.)
Yes. Please have your current treating physician, adjuster or attorney fax over the request and authorization. Our physician referral fax number is (909) 651-5484.
Please fax over the authorization letter, the demographics (claim and patient information), medical records (most pertinent – Initial/DFR (Doctor's First Report), operative reports, diagnostic reports, any Med/Legal and/or MMI (Maximum Medical Improvement) reports, and last few treating records), as well as an MPN (Medical Provider Network) list if applicable. Our physician referral fax number is (909) 651-5484.
Yes, the majority of our physicians do see Workers’ Comp patients, although most of them do not serve as Primary Treating Physicians.
