Telehealth Overview

What is telehealth?

  • Telehealth is an umbrella term that generally encompasses telemedicine, digital health, virtual care, e-Health (electronic health), and m-Health (mobile health).

The State of California defines it as follows: “Telehealth means the mode of delivering health care services and public health via information and communication technologies to facilitate the diagnosis, consultation, treatment, education, care management, and self-management of a patient’s health care. Telehealth facilitates patient self-management and caregiver support for patients and includes synchronous interactions and asynchronous store and forward transfers.” Source: CA Business & Professions Code Sec. 2290.5.

Does LLUHC have a policy for Telehealth?

Yes, LLUHC has a Telehealth Policy. Refer to PC-71.

Telehealth Modalities

  1. Synchronous Interaction: A real-time, face-to-face, two-way interaction between an individual and a health care provider using audio-visual telecommunications technology. Commonly referred to as live video or video visits.
  2. Asynchronous Interaction: The electronic transmission of medical information, such as images, documents, questionnaires, messages and pre-recorded videos through secure digitally supported telecommunications technology. The medical information transmission is initiated by an individual to a health care provider. Asynchronous services are not real-time and do not require a face-to-face interaction. Commonly referred to as store-and-forward. Examples include E-Visit and E-Consult.
  3. Remote Patient Monitoring (RPM): The use of connected digital technologies (remote diagnostic, blood glucose monitor, blood pressure cuff, etc.) to record, collect, and monitor an individual’s personal health and medical data. The individual’s data is electronically, securely transmitted to a health care provider who is prepared to review, interpret, and store the personal and medical data.
  4. Mobile Health (mHealth): The provision of technology-enabled health care services, public health information, and personal health data (wearables, health tracking app, etc.) via mobile device technologies (smartphone, tablet, laptop, etc.).

Telehealth Licensure Requirements

Physicians must be licensed in the state in which the patient is physically located at the time of visit.

Telehealth Provider Medicare Enrollment

CMS extended the flexibility requiring Practitioners to report their home address or Coleman Office on the Medicare enrollment form if they want to continue rendering telehealth services from their home until December 31st, 2024.

Telehealth Consent Requestions

At LLUH, written consent is obtained during the MyChart eCheck-In process. If the patient is sent a video link directly or unable to complete MyChart eCheck-in, verbal consent can be obtained during the visit and documented in the encounter note. If written consent is not obtained, the Telehealth Attestation is a hardstop in LLEAP.

The State of California provides the following guidance: “Before the delivery of health care via telehealth, the health care provider initiating the use of telehealth shall inform the patient about the use of telehealth and obtain verbal or written consent from the patient for the use of telehealth as an acceptable mode of delivering health care services and public health. The consent shall be documented.” Source: CA Business & Professions Code Sec. 2290.5.

Telehealth Payor Grid

Your FPBO Client Services Representative (CSM) has a copy of the Telehealth Payor Grid. The grid outlines all the available payors, CPT code, place of service, modifier, etc.

On a routine basis, FPBO, Compliance, and Digital Health meet to review the grid for updates.

Telehealth Platforms, Devices, and Data Summary

Telehealth Prescribing

The Drug Enforcement Agency (DEA) extended the temporary rule for telemedicine prescribing of controlled substances without in an in-person visit through December 31, 2024. The DEA anticipates releasing a final rule in the fall of 2024.
