Exposure/Needle Stick Reporting
In the event of an Exposure to blood borne pathogens/bodily fluids or a Needle
Stick, always wash the site and immediately notify the following:
- Direct Supervisor
- Obtain Exposure Kit (folder) with instructions and lab processing information/consent
- Injury Reporting Hotline 855-500-LLUH (5584)
- Employee Health Services, Ext 88797; 909-558-8797
The event will be triaged over the phone by the Injury Reporting Hotline RN who will direct medical care. If the RN sends the employee for treatment, the RN will instruct the employee to complete the DWC-1 form electronically. This information will be sent to the employees work e-mail.
