Restricted AntiMICROBIALS

For Pediatric Patients:

Restricted antimicrobials at LLUCH require Pediatric ID approval for use daily, weekends and holidays from 8 am to 5 pm.  Overnight (5:01 pm to 7:59 am), restricted antimicrobials that are ordered will be dispensed by the pharmacy until 12 noon the next business day.  At 8 am the following work day, prescribers need to page Pediatric ID for approval.

For Obstetrics Patients:

Restricted antibiotics in obstetrics patients at LLUCH require Adult ID approval for use daily, weekends and holidays from 8 am to 5 pm. Overnight, restricted antimicrobials that are ordered will be dispensed by the pharmacy until 12 noon the next business day, at which time an approval will be required to obtain further doses. 

To obtain approval for restricted antibiotics on weekdays, please refer to the ‘Antimicrobial Approval’ call schedule that can be found via the SmartWeb and contact the stewardship physician on call.
