Invoiceable Items Submission

The Billing and Reimbursement form is to be completed by Study PI/Coordinator or designated study administration (i.e. CTC) to provide critical information regarding study participant notification to clinicians and billing guidance to billing offices. Accurate completion of this form enables proper research billing.
