Work Injury Reporting

Reporting a Work Related Injury or Illness

In the case of a life or limb threatening injury or illness please call 911.

For all other injuries:

  • Step 1: With your supervisor, call the 24/7 Nurse Advice and Injury Reporting Hotline at 1-855-500-5584 or 1-855-500-LLUH
  • Step 2: If treatment is being sought, you will be given an email link to complete the DWC-1 form.
    • *Log on with your computer username and password​
    • As directed by the nurse advisor, you will seek medical care at one of the following locations:
      • Loma Linda University Health Care
        Occupational Medicine Center

        328 E. Commercial Road, 101
        San Bernardino, CA 92408
        (909) 558-6222 or Ext. 66222
        *Includes clean needle sticks/sharps and all Animal Care blood borne exposures
      • Loma Linda Medical Center
        Emergency Department

        Behind Medical Center on Campus St. (909) 558-4444
        *Includes all emergencies, dirty needle sticks, sharp injuries or body fluid exposures
    • Do not go to your private physician or to any other medical clinic when you have been injured or become ill due to your work.
    • If you are working in a clinical environment, please do not request consultations from physicians in your department.
  • ​​Step 3: Contact Human Resource Management within 24 hours at 951-704-1795
