Department Specific Orientation Checklists

All departments are required to provide a department specific orientation for newly hired or transferred employees to ensure orientation is consistent and meets regulatory and safety requirements. (Policy I-6 Orientation Requirements).  

Checklists

You may use one of the generic Department Specific Orientation Checklist templates to develop a checklist specific for your department. Part 1 of the checklist cannot be modified; Part 2 should be customized for your department and modified for each employee type working in your department.

  • Traveler Department Specific Orientation Checklist 

    Instructions for using the generic checklist template with your new employee:

    Part 1:  Completed within 30 days by the Manager/Supervisor with the employee to verify knowledge as it relates to specific departmental operations.  Timeframe is determined by California law and LLUH policy.

    Part 2:  Completed within 90 days by the Preceptor/Educator with the employee to verify the new employee’s competence as it relates to their specific job or role. Timeframe is determined by LLUH policy
