Faculty Medical Clinics:

Patients may be referred for hospital ID follow up and new ID consultation. For patients requiring an ID appointment, please place an ambulatory referral to the Infectious Disease FMO Clinic and call 909-558-2860 to request a follow up appointment with one of our providers. Our clinic is located in Suite 3650, 11370 Anderson St., Loma Linda, CA 92354.

SACHS Clinic

Patients may be referred for General ID consultation, hospital follow up, as well as HIV care. There is a dedicated clinic for HIV Pre-exposure Prophylaxis (PrEP). For appointments, please place an order for referral for ID to SAC/SBC or fax a referral order to 909-382-7115 and request a follow up with one of the ID providers. Our clinic is located at 250 S. G Street, San Bernardino, CA, 92410. Patients must have one of the following insurances: Medi-Cal, IEHP Direct, IEHP/Alpha Care, IEHP/LaSalle, IEHP Dual Choice, IEHP Open Access, IEHP Medi-Medi, Molina/LaSalle, Molina-Alpha Care, Medicare A&B (with or without secondary), Medicare B, Risk Management, or Self Pay

Transplant Institute

Patients with a solid organ transplant (Liver, Kidney, Pancreas) may be referred to the Transplant Institute for hospital follow up and new ID consultation. To obtain an appointment please place an ambulatory referral to Transplant clinic and in comments indicate it is for Infectious Disease consultation. Our clinic is located at 197 E Caroline Street, Suite 1400, San Bernardino, 92408.

Heart Institute

Patients with complicated cardiac infections including endocarditis, cardiac device infections, and heart transplant recipients or candidates can be referred to the Heart Institute for ID follow up. Please place an ambulatory referral to cardiology-congestive heart failure Clinic and put in the comments this is for follow up with "ID Dr. Arcobello". Our clinic is located at 11234 Anderson St, Suite 1617, Loma Linda, CA 92354.

Coccidioides Clinic at FMC

Dr. Rogstad will be seeing patients at FMC Pulmonary clinic for patients with infections related to Coccidioides. For patients needing follow up in the Coccidioides clinic, please place an ambulatory referral to the Pulmonary Clinic at FMC and in the comments indicate that it is for Infectious Disease Cocci Clinic consultation/follow up. Our clinic is located at 11370 Anderson St, Suite 3300, Loma Linda CA 92354.
