Electronic Event Reporting (EER)
The primary purpose of Event Reporting is to improve patient care by collecting information about events that occur and those that are avoided to guide improvements. All events must be reported without delay as some events may need to be reported to regulatory agencies within defined time frames. You will be presented with different questions based upon the type of event that you need to report. Please report facts (who, what, when, where, why, and how), this will help resolve events involving patients. Please be as accurate and succinct as possible.
A medical record number is required for all patient events being reported. When looking up a patient, do not enter the leading zeroes of the medical record number. Example a MRN of 000123456 should be entered into the ID number field as "123456".
Please report events as non-patient if you are unable to find patients in the EER (Midas System)
BEHAVIOR | COMMUNICATIONBehavior communication event CHANGE IN PATIENT CONDITIONUnplanned admission / return to surgery CODE LAVENDER (Peer Support)(e.g.) An EER for Code Lavender activates peer support after an adverse or traumatic event. Support is provided by a trained Peer Support team member. Additional Care for the Caregiver resources/referrals are provided when needed. DIET | NUTRITION(e.g.) NPO patient ate, wrong diet or formula, food delivered late DOCUMENTATION(e.g.) LLEAP concerns, incomplete orders, incorrect entry on medical record, issues with consents EQUIPMENT | MEDICAL DEVICE | SUPPLIESEquipment / medical device / supplies related event EXCELLENCE IN QUALITY AND SAFETY EVENT(e.g.) Action, suggestion and/or projects of an employee that enhances and/or promotes excellence in quality and safety. This event type does not replace the Loma Linda Values Care Card but establishes a manner to affirm the actions of our teams and individuals in our organization related to patient safety and quality. Examples of events submitted under this bucket could include employee interventions that solve e-consent connectivity issues or staff identification of near miss. Any act of violence or threat of violence that occurs at the work site. The term violence shall not include lawful acts of self-defense of others. Workplace violence includes the following: the threat or use of physical force against an employee that results in or has a high likelihood of result in injury, psychological trauma, or stress, regardless of whether the employee sustains an injury; an incident involving the threat of use of a firearm or other dangerous weapon, including the use of common objects as weapons, regardless of whether the employee sustains an injury. |

To report events during the downtime, please fill out the EER Downtime Reporting Form and submit the completed form to your supervisor.
