LLUCH Case Management
Our Purpose
The Case Management Department has the responsibility and accountability for coordinating care from admission to discharge and post-hospital care. Case Management, utilization review and discharge planning services are provided for inpatients. Discharge planning begins upon admission. All patients are screened for case management needs within 24 hours of admission utilizing criteria through the nursing admission and assessment.
Registered nurses monitor the utilization of healthcare services and intervene as necessary. The department provides liaison services for on-site MediCal, California Children’s Services, and other contracted health plan and medical group case managers/utilization reviewers.
