Services
The Case Management Department has the responsibility and accountability for coordinating care from admission to discharge and post-hospital care.
Our services include:
- Provision of information relating to insurance coverage to patient/families and members of the health team
- Clinical screening on admissions for medical necessity
- Utilization of the nursing process: assessment, planning, intervention, and evaluation
- Screening of all patients under the age of 21 for eligible California Children Services (CCS) conditions, and referral to CCS when indicated.
- Discharge planning including: complex patient placements, linking to home health, supplies, equipment, and education. (refer to policy CH-M-20 and procedure CH-M-20.A for details)
- Patient care rounds and chart review for clinical information on patient progress
- Collaboration with the multidisciplinary team
- Collaboration with admitting physicians
- Collaboration with third party payers
- Retrospective case review
- Complex care review for concurrent utilization and financial reimbursement
