Medical Center Policies Related to Pain Management

  • Policy M-12: Diagnostic, Therapeutic and Standing orders
    • 1.9: PRN orders
    • 1.10: Range component orders: limited to dose (e.g., 1-2 tablets)
    • 1.11: Titration orders: initial rate and parameters for titration
      • Pain medications: refer to Policy M-55
  • Policy M-55: Medication Administration and Errors
    • 11: Double checks; high-risk medications (see M-55A: pages 7-8)
      • 11.2: Prior to hanging IV
      • 11.3: Prior to dose administration
      • 11.4: initiating new or changed programming of PCA, PCEA, continuous opioid infusions
    • 12: Administering pain medications with dosage range
      • Use lowest dose in range if opioid-naïve
      • Increment to next dose as indicated, up to maximum dose
      • Notify prescriber: inadequate pain relief; adverse effects
    • 18: Discharging outpatients after receiving an opioid or sedative
