Patient Referrals

Internal Loma Linda University Health Providers may submit an “ambulatory referral” to the “PACE Program” in EPIC for patients they feel would benefit from our services and meet the criteria. This referral will go to the LLUH PACE work queue and the enrollment team will reach out to the patient within a week to screen them and answer any questions they may have.
