COVID-19 vaccine policy

The Centers for Medicare and Medicaid Services (CMS) and the California Department of Public Health are no longer requiring Healthcare Providers to receive the COVID-19 vaccination to work. However, all individuals are recommended to be up-to-date with the COVID-19 vaccination based on the Centers for Disease Control and Prevention's definition of "up-to-date." LLUH encourages all of our team members to be fully vaccinated. We will continue to monitor our vaccination rates of LLUH employees and providers in order to meet the CMS requirement for monthly reporting of vaccination percentages.

Uploading COVID Vaccine Cards

As a reminder, if you are a healthcare employee who is vaccinated, you can upload proof of vaccination by contacting Number Control at 909-558-4000 ext 58235 or email them at NumberControl@llu.edu and request that they send you the questionnaire needed to upload your vaccine card.
