Social determinants of health initiative

As part of our ongoing whole person care work, and in partnership with our regulatory agencies, all hospitals will administer a Social Determinants of Health (SDOH) screening for all inpatient hospital admissions of individuals aged 18 and above.
SDOH encompasses the non-medical factors that significantly impact health outcomes. These factors include the circumstances in which people are born, grow, work, live, and age and the broader societal forces and systems that shape overall health conditions. Recognizing and addressing these factors is crucial for advancing the health and well-being of our patients in their daily lives. By implementing SDOH screenings, we can enhance the quality of care we provide to our patients. By identifying and understanding the social risk factors associated with poor health, we can better tailor our clinical care to address these issues. Additionally, this initiative allows us to connect our patients with valuable community and government resources that can further support their well-being.
Training will be available in OWL to employees involved in the screening workflow within our LLEAP. November 27th, all units will go live with the proposed workflow so that we are ready to meet the regulation by January 2024.
